The focus
Surgery where it’s hardest to get.
We start in Nepal, Dr. Subedi’s home country, with the operations people wait longest for: burn reconstruction and backlogged general surgery. Burns alone kill an estimated 180,000 people a year, most in low- and middle-income countries (WHO).
Burn reconstruction
Contracture releases, skin grafts and scar revision that give back movement, function and the ability to work, for patients living with old burns.
Backlog general surgery
Common operations like hernia repairs and gallbladder removal that patients put off for years because they can’t afford them.
Acute burn care
Smartphone triage with AI for burn depth and %TBSA, tele-mentoring of local teams, and low-cost skin coverage training.
AI media engine
The Trenches podcasts, in every language that needs them.
Trenches in Transplant Surgery and Trenches in Burn Surgery become short clips, full transcripts and teaching lessons, dubbed into Nepali, Hindi and Spanish for surgeons and trainees who need them most.

The Founder
Dr. Sabin Subedi,
M.B.B.S., M.S.
A general surgeon from Nepal with fellowship training in burn and reconstructive surgery and acute burn and critical care (UTMB), and in abdominal transplant surgery (UTHSC and UPMC, where he was Chief Administrative Transplant Surgery Fellow).
He founded High Criteria in 2023 so that where a patient is born doesn’t decide whether they get surgery. He produces the Trenches in Transplant Surgery and Trenches in Burn Surgery podcasts.
